Providing long-term care for individuals with physical or mental disabilities presents distinct clinical and ethical challenges, particularly as clients experience age- or disease-related decline. Tailored for NGO stakeholders under the JCECC: Unison initiative, this webinar explores practical approaches to recognising early indicators of physical deterioration and frailty within this population. Individuals with complex medical needs often face heightened susceptibility to acute, life-threatening critical illnesses, which can lead to distressing emergency hospital admissions and intensive care interventions.
Drawing from intensive care medicine and clinical ethics, this session highlights how proactive Advance Care Planning (ACP) bridges the gap between community social care and acute clinical management. Participants will gain key insights into assessing physiological vulnerabilities, facilitating meaningful ACP discussions, and integrating timely palliative and end-of-life care into daily long-term care practices. By aligning treatment goals with individual preferences and quality-of-life priorities, NGO teams can reduce crisis-driven decision-making, avoid non-beneficial interventions, and uphold client dignity. Ultimately, this presentation aims to empower frontline practitioners to guide clients and families in making informed, person-centred care choices as their health trajectories evolve.
